DJ Performance Application

This form is for the purpose of applying to perform at events hosted by "Club Kaiju". These events are categorized as "Anime Rave" events that cater to the anime + pop culture community and nightlife. This application is for PERFORMING DJS ONLY. No rappers, band performances, dance acts, or anything that involves acts not utilizing DJ equipment. Any and all applications that do not abide by this rule will be thrown away/deleted from our roster.
